Medical Summary DVM Intake Exam Estimated age: Approx 9 weeks old Microchip noted on Intake? scan negative History : stray with 3 other kittens and mother cat Subjective: Observed Behavior – hissing, trying to flee Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 5/9 EENT: OU- green mucoid discharge and conjunctival hyperemia, AU- mild brown debris, no nasal discharge noted Oral Exam: deciduous teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment conjunctivitis- R/O infectious vs other Plan start erythromycin OU BID x 10 days start amoxi/clav 15 mg PO BID x 10days Prognosis: good SURGERY: Temporary waiver due to URI
