History : stray cat found lying in front of building; appeared injured Subjective:QAR Observed Behavior – hiding in back of kennel; a little unsocialized but allows handling Evidence of Cruelty seen – none Evidence of Trauma seen – facial swelling, traumatic conjunctivitis Objective BCS 4.5/5 EENT: OU-moderate blepharitis and severe blepharospasm; intermittently will open eyes slightly, elevated 3rd eyelid, mild mucoid discharge; mild abasions along lids no nasal discharge ears clean Oral Exam: mm pk, tacky; CRT <2 sec; mild tartar, moderate staining; no mandibular fracture or soft palate trauma PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated; soft/normal fecal material palpable in colon U/G:male intact, L testicle twice as large as R MSI: appears ambulatory x 4 with no obvious lameness but prefers to curl up moderate facial swelling possible mild bilateral soft tissue swelling of antebrachium mild crusting dermatitis along back of head hypotrichosis along shoulder blades with mild crusting-wood’s lamp neg mild muscle wasting of hind limbs CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Traumatic conjunctivitis Facial swelling Mild dermatitis MIld dehydration Testicular asymmetry Plan sedated with 0.2 ml torb/0.2 ml dexdomitor for full evaluation Onsior 20 mg/ml: gave 0.4 ml SQ, continue SID x 2 days Simbadol 1.8 mg/ml: gave 0.5 ml SQ, continue SID x 5 days Convenia 80 mg/ml: gave 0.4 ml SQ LRS: 150 ml SQ, continue SID x 3 days Erythromycin OU BID x 7 days Prognosis: good; suspect possible high rise trauma with no obvious fractures SURGERY: Okay for surgery
