Medical Summary 05/20/17 10:42 DVM Intake Exam Estimated age: approx 2-3 yrs Microchip noted on Intake? no History : stray admitted to SiACC last night, large wound and dirty stained haircoat noted, received – 0.4ml convenia SQ – 0.55ml simbadol SQ – 100ml LRS – Flushed both wounds and cleaned with nolvasan – Applied bandage around lower abdomen Subjective: Observed Behavior – very energetic and friendly, allows full exam Evidence of Cruelty seen – possibly, haircoat convered with sticky substance and purple coloration Evidence of Trauma seen – yes, wound on ventral abdomen Objective T = P = 240 R = 24 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild dental tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted,, haircoat is filthy with sticky black substanecas well as purple coloring in patches, large amount of pasty light-brown diarrhea adherent to hindlimbns and tail, approximately 3cm diameter full thickness granulomatous purulent wound on ventral abdomen, abrasion/.wound on medial RFL at carpus, and laceration at lateral portion of digit 5 of LHL involvig pad, torn claw on LFL digit 2 CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment wound dehydration diarrhea Plan 100ml LRS SQ BID x 2 days clip matted fur, clean/bathe flush wounds 1.23ml Metronidazole PO BID x 5d 2.0ml Ponazuril PO SID x 3d 0.55ml Simbadol SQ SID (in pm) x5d sedate for wound closure and neuter in 2 days Prognosis: good SURGERY: Okay for surgery
