Subjective:
Observed Behavior – BARH. Allows all handling, attention seeking, eating well, diarrhea
Evidence of Cruelty seen – No
Evidence of Trauma seen – No
Objective
P = WNL R = WNL BCS 4/9
EENT: Eyes clear, conjunctivitis OU, ears clean, moderate serous nasal discharge
Oral Exam: Teeth clean, pink mm, no oral ulcers
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Female, lactating
MSI: Ambulatory x 4, dirty hair coat, fecal staining, erythema around hindlimbs/hindend
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: URI, conjunctivitis, lactating, diarrhea
Plan: Rec doxycycline 0.7ml PO q24 x 10 days, erythromycin OU q12 x 10 days. Consider metronidazole if diarrhea doesnt improve with deworming
Prognosis: Good

Temporary waiver due to URI and nursing
