Medical Summary DVM Intake Exam Estimated age: Approx 8 years old Microchip noted on Intake? scan negative History : Subjective: Observed Behavior – hissing and tense but tolerated exam Evidence of Cruelty seen – no Evidence of Trauma seen – abrasion at nares Objective BCS 5/9 EENT: Eyes clear, AU- mild brown debris, no nasal discharge noted Oral Exam: limited oral exam; severe dental calc and gingivitis seen PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, mild tachypnea ABD: Non painful, no masses palpated U/G: MN MSI: Ambulatory x 4, skin free of parasites, no masses noted, superficial abrasion at nares CNS: mentation appropriate – no signs of neurologic abnormalities Assessment severe dental disease abrasion at nares- R/O rubbing on carrier vs other mild tachypnea- R/O stress (most likely) vs asthma vs other Plan recommend dental monitor respiration Prognosis: fair
