10/14/17 DVM Intake Exam Estimated age: ~3 months Microchip noted on Intake? No History : Owner surrender, owner reported anorexia for the past 3 days Subjective: Observed Behavior -QAR. Mild dehydration. Allows all handling. No interest in food Evidence of Cruelty seen -No Evidence of Trauma seen -No Objective P =WNL R =WNL BCS 4/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: Pink mm, teeth erupting normally PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: Recent onset of anorexia, dehydration Prognosis: Fair Plan: Parvo test neg. Rec LRS 30ml SQ q24 x 2 days, metronidazole 0.15ml PO q12 x 5 days, clavamox 0.3ml PO q12 x 10 days, nutrical q12 x 3 days, mirtazapine 1/8 tab PO — first dose give of all. force feeding by assistant at time of exam. thoracic radiographs unremarkable SURGERY: Temporary waiver due to anorexia
[Spay/Neuter Waiver – Medical Condition] Your newly adopted pet has been diagnosed with anorexia and the staff veterinarians are issuing a temporary waiver from the spay/neuter requirements of the City of NY. Follow up care at your regular veterinarian is recommended to ensure continued treatment. Your veterinarian will advise you if surgical sterilization is appropriate.
VC: Monitor condition – ADR kitten S/O: Kitten is dull/depressed, dehydrated. Urine++ in litterbox and water dish is empty. Does not appear to be eating, may have licked at food. T – 99.0 F P – 200 R – wnl mm – light pink, tacky A: Approx 10-12% dehydrated (increased skin tenting, tachycardia, sunken eyes dry corneas, alteration of consciousness), Lethargy, Anorexia/Inappetence, PU/PD? DDx: ARF, Open P: Give 30 mLs SQ LRS and wait 60 mins before placing IV catheter and taking blood for CS and PCV/TS. Addendum: LVT placed 22G IV catheter right cephalic and started on IV fluids at 9mls/hr. Bloodwork reveals severe azotemia, suspect pre-renal and renal. PCV 37% and TS 9.2. Continue fluids and recheck BUN/Crea in 24-36 hours. Also gave 0.12mLs Convenia SQ (for possible UTI/pyelonephritis), 0.12 mLs Cerenia and 0.06 mLs Famotidine SQ for anorexia/inappetence. Force feed prn. Added vet check for 10/16/17. Prognosis: Poor-Guarded depending on response to fluid therapy and supportive care. Dr 1167

CXR-wnl Parvo snap-negative 10/15 CBC-PCV 37%, TS 9.2 Chem-hyperalbuminemia 4.3 (2.2-3.9), severe azotemia (BUN >130 (16-33), creatinine too high to read), hypercholesterolemia 311 (62-191), hyperglycemia 223 (77-153), hyperphosphatemia 12.3 (4.5-10.4), hyponatremia 124 (150-165), hypochloridemia 92 (115-126) Started on IVF LRS 180ml/kg/day and gave convenia, cerenia, and fomotidine 10/17 Chemistry-mild hypercholesterolemia 232 (62-191), elevation GGT 3 (0-1), hyperglycemia 158 (77-153), mild hypokalemia 3.4 (3.7-5.9), mild hypochloridemia 114 (116-126) Subjective: QAR, euhydrated.
