SAFE 8/8/17 COLLOSUM is a little kitten that has Ringworm, URI, Conjunctivitis, Underage, & Underweight. He needs an immediate foster home so he can receive some TLC.

History : P presents as strayl. Very vocal allows all handling.
Evidence of Cruelty seen – No
Evidence of Trauma seen – No
P = 220 R = 50 BCS 4/9
EENT: Chemosis, green ocular discharge OS, Clear OD , ears clean, Crusting with green nasal discharge
Oral Exam: decidious dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: intact male
MSI: Ambulatory x 4, Flea dirt noted, no masses noted, Circular area of crusting above base of tail as well as on abdomen
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: Fecal staining noted around rectum
Assessment: Ringworm, URI, Conjunctivitis, Underage, Underweight
Erythromycin Ophthalmic OU BID x 10 days
AmoxiClav 45.7 mg/ml- 0.06 ml PO BID x 10 days
Syringe feeding KMR
Parvo SNAP- negative
Temporary waiver due to underage, underweight
