I came in the shelter as a STRAY on 03/29/2016 from NY 11208, owner surrender reason stated was TNR. I came in with Group/Litter #K16-051807.

Scanned negative QARH – shy, nervous but “talks”, crouches in cage, may flee. Due to illness, gave 0.1mL Telazol for exam. F~ 18 months (shaved no spay scar) Eyes – heavily crusted with yellow discharge – cleaned and applied Eryc oint. Nose, yellow-white discharge, sneezing. Throat clear Teeth – heavy tarter, gingivitis. Halitosis. Discussed with Vet #1173 – begin treatment for URI. Coat clean – no flea dirt or parasites noted ambx4 NOSF
