Medical Summary DVM Intake Exam Estimated age: 4-8 years old based on dentition Microchip noted on Intake? Scanned negative, MC placed on intake exam History : Stray – was sitting outside finder’s home for a day, not moving Subjective: Calm, walks out of carrier, allows all handling Evidence of Trauma seen – Tachypneic Objective P = 140 R = 64 BCS 3-4/9 Approximately 5-8% dehydrated – decreased skin turgor EENT: Eyes clear, ears clean, small amount of black dry debris under the nose Oral Exam: Missing most incisors, right max and mandib canine teeth are chipped PLN: No enlargements noted H/L: HR 140, NSR, NMA. RR 64, harsh lung sounds on left side of chest, quieter lung sounds on right side of chest. Abdominal exertion. ABD: Tucked up, no masses palpated U/G: Male intact, cryptorchid MSI: Ambulatory x 4. Large amount of flea dirt. Thin fur coat, patchy alopecia on dorsal neck and over shoulders. No masses noted. CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Not performed Assessment: 1. Tachypnea, unilateral harsh lung sounds – R/O pneumothorax vs. other 2. Flea dirt, thin fur coat/alopecia 3. Underweight 4. Cryptorchid Plan: 1. Chest rads immediately Prognosis: Fair SURGERY: Decide based on results of rads 1088
