History :stray cat found in street with signs of head trauma; was given simbadol, onsior, mannitol and started on IVF at 10 ml/hr
Subjective:QAR
Observed Behavior -skittish, sl unsocialized but allows gentle handling; mild euphoria secondary to simbadol
Evidence of Cruelty seen -n
Evidence of Trauma seen – partial proptosis OD, facial swelling, traumatic rhinitis
Objective
BCS 5/9
EENT: OD-partial proptosis, cornea appears dull, non visual; OS-enophthalmia, moderate blepharospasm and conjunctivitis, globe appears intact, ears clean, crusting bloody discharge along nares, nasal congestion
Oral Exam:mm pk, tacky; open mouth breathing secondary to nasal congestion; moderate tartar/staining on teeth; upper canines fractured; moderate swelling of tongue with wound at tip, tissue along dorsal aspect of tongue appears tan/pale, may represent necrosis
PLN: No enlargements noted
H/L: clear lung sounds, no murmurs/arrhythmias
ABD: Non painful, no masses palpated
U/G: male intact
MSI:
-ambulatory x 4 with no obvious lameness or injuries on body; skittish so difficult to fully evaluate gait
-coat slightly damp
CNS: quiet but appropriate mentation
Rectal:grossly normal
Assessment
1. Head trauma, r/o high rise vs HBC
2. Proptosis OD
3. Traumatic conjunctivitis OS
4. Traumatic rhinitis
5. Tongue wound
Prognosis:fair to good
Plan:
-d/c onsior, gave dexamethasone SP 4 mg/ml: 0.25 ml IV
-continue IVF at 10 ml/hr
-ofloxacin eye drops OU TID x 10 days
-cerenia 0.4 ml IV SID x 3 days
-convenia 0.4 ml SQ today
-simbadol 0.4 ml SID x 4 days
-nebulize SID x 3 days
-recommend enucleation OD once stable enough for anesthesia
SURGERY:temporary waiver due to head trauma
