SAFE 6/1/17 **Adorable 6-Week Old Gray/White Kitten – CHLOE – With URI Needs You!**
Chloe A1113711 – 6 week female gray/white kitten:
DVM Intake Exam
Estimated age: 6 weeks
Microchip noted on Intake? none
History : Stray, found wandering in yard – possible URI.
Subjective: BAR- H.
Observed Behavior – Approached front of cage, head butting, purring when pet. Ate kitten food well.
Evidence of Cruelty seen – none
Evidence of Trauma seen – none
T = P = 200 R = BCS=5/9
EENT: Eyes – OS – mild MPOD, cornea/sclera/conj clr. OD- mod chemosis and conj hypermia, mod MPOD. Ears – black discharge AU, pedal reflex when cleaning ears. MPND bilateral. Congested.
Oral Exam: No ulcers, all baby teeth present.
PLN: No enlargements noted
H/L: NSR, NMA, MM pk, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated but pot bellied abd.
MSI: Ambulatory x 4, skin free of parasites, no masses noted, dry haircoat but no skin lesions.
CNS: mentation appropriate – no signs of neurologic abnormalities
6 week female kitten.
Ear mites AU.
Cleaned ears. Placed revolution for mites. No other flea tx placed.
Gave doxy 0.1 cc po. Start Clavamox 0.15 cc po bid x 10 days.
Cleaned eyes. Erythromycin eye oint OD BID x 7 days.
Temporary waiver due to underweight, under-age and URI.
