Medical Summary DVM Intake Exam Estimated age: 3yrs Microchip noted on Intake? neg History : o surrender Subjective: very sweet, docile Observed Behavior – allows all handling, docile Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 180 R = 60 BCS 7/9 EENT: Eyes clear, ears moderate dark aural debris, pruritic, no nasal discharge noted Oral Exam: pink gums, mild gingivitis PLN: No enlargements noted, prominent popliteals bilaterally- r/o fat surrounding llnn making them palpate as prominent H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FS, grossly normal, no palpable MGTs MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment overweight mild periodontal dz AU ear mites Plan ok for adoptions gave ivermectin SQ on intake, repeat in 14days clean ears long term rec weight loss through controlled feedings and exercise at home- ideal weight closer to 10 lbs, can be safely lost over approx 4 months. Long term rec brush teeth every other day long term if p allows with cat toothpast and brush to promote oral health Prognosis: good SURGERY: N/A- p already spayed
