Intake condition EXAM REQ Intake Date 03/13/2017, From NY 11204, DueOut Date 03/16/2017, I came in with Group/Litter #K17-090874.
Chili was brought in as a stray, so we cannot speak to her behavior in her previous home.
Friendly easy to handle
Comes right to the front and is calm, relaxed, and friendly. Arches back when touched, head-butts, leans in, and purrs. Very sweet and friendly.
Cage Condition: Cage slightly re-arranged
Reaction to assessor: Chili was relaxed and eating her dry food at the front of the kennel.
Reaction when softly spoken to: Chili makes eye contact then meows softly and rubs her cheeks up against the kennel door.
Reaction to cage door opening: Chili is calm and relaxed.
Reaction to touch: Chili chirps then leans in for attention, allowing petting and rubs all over her body. She’s incredibly affectionate and is curious of her surroundings.
Reaction to being picked up: Chili is calm, relaxed and allows all handling.
ACTIVITY LEVEL: Moderate, and another if applicable
VOCAL: Somewhat chatty
CHARACTER TYPE: Curious, Sweet, Affectionate
Beginner – Chili interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.
S/O: BARH. At front of cage meowing. All food and water has been consumed. No vomiting/diarrhea.
EENT: No oculonasal discharge.
MS: Amb x 4, no lameness or weakness.
A: Excellent appetitie – Polyphagia and polydipsia secondary to hyperthyroidism.
P: Offered fresh food and water.
Continue to monitor while at BACC.
Good prognosis with appropriate treatment.
S/O: QAR. Purring. Eating with excellent appetite. Clean litter
A: Appears stable at this time. Geriatric, underweight
P: CBC/Chem pending. Continue to monitor while at BACC. Open prognosis depending on underlying disease
S/O: BARH. mm=pink, moist, CRT<2 s. Very friendly and social, eager for attention. Water bowl empty, has eaten all of canned food.
ORAL: Mod. to severe calculus and gingivitis. Missing multiple teeth. R max C missing, gingiva in this area is very inflamed, sensitive.
EENT: Iris atrophy OU. Eyes sunken OU. No oculonasal discharge.
H/L: HR=220, RR=purring. Lungs clear, no murmurs/arrhythmias (but cat is purring loudly).
ABD/UG: Soft, non-painful. No masses/organomegaly.
MS: Amb x 4, no lameness. Marked generalized muscle wasting. BCS=1/9.
INTEG: Full, clean haircoat. Palpable thyroid slip.
A: Age est 14 years. Polydipsia with emaciated body condition – r/o renal disease, hyperthyroid, diabetes mellitus, neoplasia, other.
P: Run CBC/CHEM/T4 for further workup.
Open prognosis pending lab results.
