**SAFE 08/18/2017** BEGINNER RATED AND ONLY A YEAR OLD….but it may not have been the best of years. Poor Chester was brought into the shelter by someone who felt he was injured. Chester is ringworm positive, has a URI and a lesion on his tongue which needs attention. Are YOU the person who can turn things around for Chester? ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Sweet, Affectionate MEDICAL BEHAVIOR: Observed Behavior – tail up, very social ENRICHMENT NOTES: 8/10/17- Resting on bedding, soft eyes and body. Started meowing on approach, came to the front. Leaned and arched into pets, looked around. Very friendly and curious. EVALUATION: Cage Condition: No change Reaction to assessor: Chester immediately comes soliciting at the front of the cage. Reaction to door opening: Chester remains standing at the front of the cage, relaxed body posture, soliciting attention. Reaction to touch: Chester head-butts the assessor’s hand and appreciates petting on the head and body. Reaction to being picked up: Allows the pickup and remains calm. BEHAVIOR SUMMARY: Beginner Chester interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.

Observed Behavior – tail up, very social Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 200 R = wnl BCS 4/9 EENT: OU purulent eye d/c, eyes slightly enopthalmic Oral Exam: lesion on rostral tongue, right lateral commissure of lips. ptylalism PLN: peripheral lymphadenopathy. popliteal esp—other ln much more subtle H/L: NSR, NMA, , Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: URI, ringworm. possible calici virus. Plan: intrafungol 1.9ml sid x 28 days doxy 0.9ml sid x 10d erythromycin ou bid x 10d Prognosis: fair–will need treatment for 4+ weeks SURGERY: Permanent waiver due to ringworm/uri

08/12/17 11:00 S/O: BAR, usually very friendly after getting itrafungol, started to vocalize and staff unable to medicate doxycyline in the mouth BAR, eating well, exam unchanged since yesterday- only visual since p now temporarily agitated A: URI with conjunctivitis MI r/o ringworm r/o calici P: cwsc 8/11/17 S/O: URI, possible ringworm, possible calici offered canned- eating with gusto VERY FRIENDLY, allows all handling BAR, amb x4, BCS 5/9 M/S: R lateral neck- reddered alopecic lesion, also alopecia on R lateral neck rostral to that, very mild erythema at pre auricular areas bilaterally.
