BCS 4/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: mild to moderate dental tartar, no ulcerations
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: seemed mild to moderately uncomfortable on mid-abdominal palpation, no masses palpated
U/G: intact male with two descended testicles
MSI: Ambulatory x 4, skin free of parasites, no masses noted, roughened hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: normal externally
Assessment
1) Anorexia, vomiting, diarrhea- Given age and acute on chronic nature (that we know of), a more chronic GI disease process is suspected. Top differentials would be pancreatitis vs IBD/lymphoma. However, given his abdominal pain, an acute process such as an intestinal obstruction cannot be completely ruled out. Other systemic causes such as liver disease (cholangiohepatitis, toxin, neoplasia), renal disease (CKD, acute on chronic renal disease ) and systemic neoplasia are also possible. Less likely gastroenteritis secondary to dietary indiscretion, infectious.
2) FIV+
3) Anemia, regenerative- r/o secondary to #2 vs infectious (mycoplasma haemofelis) vs IMHA (primary vs secondary to vaccines, infectious, neoplasia) vs machine error (and really non-regenerative secondary to anemia of chronic disease or neoplasia)
Prognosis: Fair to guarded long term
Plan:
CBC/Chem/T4
CBC- Regenerative Anemia noted (Hct 25.6%, Retic 327.6)
Chemistry- unremarkable
T4- normal
Saline-agglutination test- negative for micro and macro agglutination
Abdominal radiographs (two view): stomach appears small, intestines appear diffusely small and fluid filled
Blood Smear made- read tomorrow
CTM in Medical
LRS 80mls SID given once
Cerenia 1 mg/kg SQ SID x 3 days then re-eval
Simbadol 0.24 mg/kg SQ SID once, then re-eval
Metronidazole 10 mg/kg PO BID x 5 days, then re-eval
SURGERY:
