Medical Summary DVM Intake Exam Estimated age: reported to be 5 Microchip noted on Intake? neg History : owner surrender due to owner’s living situation; owner reports chaz has been scratching on the walls on inappropriately urinating around the house Subjective: QAR Observed Behavior – docile, allows handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 9/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, moist; CRT <2 sec; mild tartar/staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered male MSI: Ambulatory x 4, skin free of parasites, no masses noted, coat starting to matt over dorsum; dandruff CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Obese Dandruff-suspect undergrooming due to BCS Plan rec’d weight loss through dietary management; due to severe obesity, a prescription diet such as Hill’s metabolic is recommended and weight loss should be monitored closely monitor appetite Prognosis: good SURGERY: neutered
