Charlotte – A1107133 (3 yr. female black/white mom) & her 3 kittens:

By the back, calm and relaxed. Meows when spoken to and approaches the front. Accepts petting, lifting her rear and rubbing her head against me. Attempts to climb on my lap and nuzzle. When petting stops, she curls up by the back and lies down. Seems a bit unsure of environment but is very sweet.
DVM Intake Exam Findings
3 year age
History: The patient is stable. No vomiting, diarrhea, coughing or sneezing. Eye ulcer and lactating.
Subjective : Queen, Lactating,
Observed Behavior – Do not tolerate anybody touching the face.
Evidence of Cruelty seen – None
Evidence of Trauma seen – Right eye
T =Not taken P = 160 bpm R = 32 bpm BCS 3/9
EENT: Deep corneal ulcer( conjuctivitis and yellow discharge) on the rigth eye, ears clean, no nasal discharge noted.
Oral Exam: mild gingivitis
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated, lactating
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: Not performed
FELV/FIV- felv positive, FIV negative
Medical Behavior Status- blue
Medical Status- 4nc
Asilomar Status- Unh and untreat
Fast Track- None
Rabies and FVRCP vaccine given

0.7 ml Amoxicillin and clav BID PO for 21 days
