Medical Summary S/O: relinquished by o, as per o blocked for at least a day prev MR: brought to VERG 8/20/16 for hematuria given 100ml LSR SQ and sent home with clavamox, prazocin, and buprenorphine MC: negative, none scanned limited exam, growling obese ABN: painful, moderately sized hard bladder unable to perform rest of exam A: UO P: unblock prognosis: fair- can recur; rec urinary based diet ideally canned such as Hill’s C/D or RC S/O; decrease stress, annual UA’s. Strongly rec weight loss unblock- sedated with telazol 2.0ml IM difficulty extruding penis, attempted to pass tomcat catheter, IV catheter without stylet, was finally able to flush with 3.5F red rubber and pass, lateral abn radiograph confirmed appropriate placement in bladder, no uroliths seen once awake will start on pain management: buprenorphine 0.5mg/ml oral solution 0.3ml PO BID x5 days(based on lean weight of 18#, at 0.02mg/kg dose) prazocin 1mg: 1/2 T PO BID x7 LRS plus 20meq/L KCl at 1.5x maintenance: 30ml/hr will pull Urinary catheter once urine in collection bag is clear, then monitor for urination Likely obesity is major contributor to UO; strongly rec weight loss
