SAFE 5/28/18 CATTY was found on the rough streets of Brooklyn. Stray intake 5/23 has multiple chronic wounds of varying ages across body. Very dirty. Sedated for rads and wound treatment of dorsal caudal tail base area-left wounds open to drain. Started on clavamox, simbadol, and onsior. Gave ivermectin for ear mites.
Radiographs: pelvis appears normal with no fractures, RHL nsf, LHL the tibiotarsal joint is completely dislocated, no fracture or remodeling noted. Please help sweet Catty today.

History: Stray intake 5/23-multiple chronic wounds of varying ages across body. Very dirty. Sedated for rads and wound treatment of dorsal caudal tail base area-left wounds open to drain. Started on clavamox, simbadol, and onsior. Gave ivermectin for ear mites.
Radiographs: pelvis appears normal with no fractures, RHL nsf, LHL the tibiotarsal joint is completely dislocated, no fracture or remodeling noted
Subjective: BAR, ~5-6% dehydrated. Eating vigorously. No csvd. Normal BM and U. Appears stable at this time but will need stabilization of his LHL ASAP.

EENT: clear eyes, clean ears, no nasal d/c
Oral Exam: adult dentition with mild dental disease
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: MI, 2 testicles descended
MSI: Ambulatory x 4 with LHL minimally weight bearing lameness-able to palpate the dislocation of the tibiotarsal joint. Eosinophilic granulomas on face and upper lip, scar on dorsum ~5cm long, multiple puncture wounds at dorsal caudal tail base-healthy granulation bed with no active d/c, appears comfortable, very dirty haircoat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment:
Multiple puncture wounds at dorsal caudal tail base
Dislocation of left tibiotarsal joint
Eosinophilic granulomas
Mild dehydration
Prognosis:good
Plan:
Continue clavamox until 6/2
Continue simbadol until 5/28
Continue onsior until 5/25
Give 100ml SQ LRS today
Scheduled bloodwork for tomorrow
Recheck wounds and overall condition daily-may need wound closure in the future.

Cat was sedated with telazol 0.06ml IM and maintained on isoflurane. The ears were thoroughly cleaned. The area above the tail base was shaved and the wounds were scrubbed. There were ~10 punctures varying from ~3mm-~1cm in side that all communicated. The wounds did not have any active discharge but had a thick scab over them which was scrubbed off. The wounds were flushed thoroughly with sterile saline and the edges were debrided with a #10 blade. The wounds were left open to continue to drain. E-collar placed. Recheck daily-may need closure in the future.

Subjective: abrasion on forehead, area of alocpecia (old road rash ?) on dorsal midline. cursted scabby injury at dorso sacral area. tail can move normally. LHL hock/tibia appears to have old fused fracture

MSI: Ambulatory x 4 but appears weak on hind limbs, skin free of parasites, no masses noted, healthy hair coat

Assessment: possible HBC, conerned about pelvic injury. open lesion at base of tail. poor muscle tone. ear mites. old fracture lhl
