The veterinarian is hereby issuing a permanent spay/neuter waiver, from the spay/neuter requirements of the City of NY due to the estimated age of this animal. ACC does recommend you consult with your veterinarian to determine if surgical sterilization is appropriate.

Oral Exam: Moderate staining on all arcades
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: Vulva UR – intact female
MSI: Ambulatory x 4, skin free of parasites, no masses noted, hair easily falling out, matted/greasy on hind
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Externally normal

Plan:
1) Doxycycline 10 MG/KG SID PO for 14 days – 0.35 ml
2) Sub Q fluids 100ml SID for 4 days
3) Cerenia 1mg/kg 0.3 sq SID for 4 days
4) Fortiflora 1/2 sachet with wet food
5) Vit B 0.3 ml SQ once only
6) Nebulize with saline SID for 5 days

Permanent waiver due to age and illness

ENRICHMENT NOTES:
Severe URI. Resting in litter box, became alert on approach, sat up, eyes fully dilated. Retreated when door opened, then tried to sniff hand, warmed up immensely. Leaned and arched enthusiastically into pets, purred loudly, started to roll over. Relentlessly affectionate.
