Medical Summary DVM Intake Exam Estimated age: 1yr Microchip noted on Intake? neg History : stray, brought in by passerbyers who applied bandages on HL wounds Subjective: Observed Behavior – allows all handling but started to vocalize and swat tail when HLs manipulated Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective HR=200 RR=48 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, moderately sized, non painful pliable bladder U/G: MI both testicles descended, normal consistency MSI: Ambulatory x2, some movement in LH limb, p losing patience during exam- no visible movement in R LH seen, superficial wound on pedal surface of LH paw and dorsal aspect of L HL, skin free of parasites, no masses noted, alopecia on caudal aspect of AS pinna, tiny pieces of debris on fur, no skin lesions otherwise, slightly dirty fur coat. Some anal tone present. Unable to test for deep pain in RH as p lost patience with exam and intake procedures CNS: mentation appropriate Assessment R pelvic fractures MI wounds on HL paws secondary to lameness- no obvious infection Plan 2v HL rads- R pelvic fractures- unable to fully assess if there is spinal involvement overlying the pelvic fractures, feces in colon check urination, check defecation, check app buprenorphine 0.3mg/ml sol : 0.4ml IM at 4:30PM, then transmucosally 0.4ml at side of cheek BID x5 if p can urinate and defecate on his own, pls contact NH for placement offered canned food and ate with gusto Prognosis: open- depending on whether can be cared for by someone who would be willing to take care of p that cannot walk on all 4limbs, is prone to pressure sores vs orthopedic consultation and possible surgery SURGERY: Permanent waiver due to pelvic fractures
