Medical Summary DVM Intake Exam Estimated age: 3-4 years Microchip noted on Intake? none History : owner surrender Subjective: QAR Observed Behavior – shy but very sweet; very easy to handle and examine Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = not taken P =WNL R =WNL BCS 5/9 EENT: OU-mild periocular dermatitis, mostly below eyes with mild mucoid discharge; ocular structures WNL AU-mild waxy discharge at external canals nose clear Oral Exam: mm pk, moist; CRT <2 sec; fractures of both upper and lower R canine; enamel defect of upper L canine; mod tartar and gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered male MSI: Ambulatory x 4, skin free of parasites crusting, mild staph collarettes along caudal dorsum with mild pruritus CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. dermatitis-suspect allergies; r/o fleas vs environmental vs food 2. periocular dermatitis-secondary to brachycephalic vs allergies 3. tooth fractures/dental dz Plan convenia inj for dermatitis rec’d dental with poss extractions Prognosis: good SURGERY: neutered
