I came in the shelter as a OWNER SUR on 11/05/2016 from NY 11215, owner surrender reason stated was OWNER HOSP. I came in with Group/Litter #K16-080522.

11/14/2016 Exam Type RE-EXAM – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is BEGINNER, Weight 7.1 LBS.
11/14/16 Eye discharge noted on rounds. O: BARH. Eating well. EENT: OU – mucoid discharge, brown crusting at medial canthus, no chemosis. Very mild crusting at nares with serous discharge. No sneezing during exam, H/L: Lungs clear. A: Mild conjuctivitis/URI. P: Move to iso. Rx Terramycin ointment. Apply OU BID x 7 days. Rx Doxycycline 50 mg/ml. 0.75 ml PO q 24 hours x 10 days. Excellent prognosis.

11/08/2016 BEHAVIOR EVALUATION – BEGINNER
Exam Type BEHAVIOR
Calico was brought in with limited information on her behavioral tendencies in her previous home environment. She has lived with two other cats and was receptive to handling during intake. The medical team reports she remained by the front of her kennel, but had wide eyes and appeared like she wanted to flee. She showed signs of stress from unfamiliar noises. The enrichment facilitator reports she was by the front of her kennel when approached, but was stress meowing and tense during their first interaction. She allowed head rubs, but was a little sassy when touched on her lower back. During her second day, she appeared to have adjusted pretty well. She comes forward and appreciates treats, allowing all petting on her head and body. Reaction to assessor: Calico was calm, relaxed, and friendly by the front. Reaction when softly spoken to: Calico meows and reaches out gently with her paw. Reaction to cage door opening: Calico remains soft and relaxed. Reaction to touch: Calico head-butts and begins to purr right away. She leans in for petting and is very affectionate throughout the interaction. Reaction to pick up: Calico remains soft and continues to purr when held. She was hesitant to be placed back in her kennel, but once placed back down she continues to solicit attention. Behavior Determination: Beginner Calico interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.
