DVM Intake Exam
Estimated age: 3 weeks.
Microchip noted on Intake? No.
History : Litter of 3 stray kittens, noted to have URI.
Subjective:
Observed Behavior – Quiet, a little fearful, allows all handling.
Evidence of Cruelty seen – No.
Evidence of Trauma seen – No.
Objective: QAR. mm=pink, sl. tacky, CRT<2 s. HR>240, RR=28. BCS=3/9.
ORAL: All deciduous teeth erupted. No oral lesions
EENT: Mild mucopurulent ocular discharge, chemosis. Mild mucopurulent nasal discharrge, congestion, sneezing.
PLN: No enlargements noted.
H/L: Lungs clear, no murmurs/arrhythmias, no dyspnea.
ABD: Mod. pot-bellied appearance. No masses/organomegaly.
U/G: Female.
MS: Ambulatory x 4, no lameness.
INTEG: Full, unkempt haircoat, mild decreaesd skin turgor. No skin lesions, no fleas.
CNS: Mentation appropriate – no signs of neurologic abnormalities.
A: Underage, underweight kitten with URI, conjunctivitis, mild dehydration.
P: Rx Amoxi-Clav 45.7 mg/ml. 0.2 ml PO BID x 10 days. first dose given.
Rx Erythromycin ointment OU BID x 10 days. first dose applied.
10 ml LRS SQ q 24 hours x 3 days.
Temporary waiver from OVH surgery due to underage, underweight, and URI.
Good prognosis.
Rec. foster placement.
