Medical Summary DVM Intake Exam Estimated age: 4-6 weeks Microchip noted on Intake? negative History : stray Subjective: BARH Observed Behavior -Friendly and good for all medical handling Evidence of Cruelty seen – no Evidence of Trauma seen – yes, wound on back Objective P = wnl R = wnl BCS 4/9 EENT: No d/c ou, clean ears au, no nasal discharge noted Oral Exam: kitten dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated, distended U/G: FI MSI: Ambulatory x 4, skin free of parasites, no masses noted, ~2cm circular wound between the shoulder blades with a healthy granulation bed, no d/c from wound CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Wound between shoulder blades Underage Plan: Clipped and scrubbed wound Recheck SID x5d-if no improvement or starts to have d/c, consider adding abx Prognosis: good SURGERY: temporary waiver for surgery due to underage/underweight
