KNOWN HISTORY: Butterfly was brought in as a stray, so there is no information on her behavior in her previous home. Her finder reports she walked onto their porch and would not leave. MEDICAL BEHAVIOR: Observed Behavior – friendly at first but growling while examining wounds. EVALUATION: Cage Condition: Cage is messy Reaction to assessor: Butterfly Approaches, body soft. Reaction when softly spoken to: Butterfly rubs on cage doors. Reaction to cage door opening: Butterfly remains soft and relaxed. Reaction to touch: Butterfly solicits petting and attention. Reaction to being picked up: Butterfly allows to be picked up but hisses after placed back into her kennel. ACTIVITY LEVEL: Moderate VOCAL: Quiet CHARACTER TYPE: Social, Sweet BEHAVIOR SUMMARY: Please note that this cat is being treated for a medical condition at the time of evaluation. It is difficult to determine at this time how the medical condition may be affecting the behavior. Butterfly interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. In spite of having a wound, she remains social and affectionate. This cat is showing behavior appropriate for new or experienced cat parents.

DVM Intake Exam Estimated age: 8 years History : stray Subjective: Observed Behavior – friendly at first but growling while examining wounds Evidence of Cruelty seen – no Evidence of Trauma seen – wounds along dorsum Objective BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm’s pale pink, mild dental calc PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: F MSI: Ambulatory x 4, skin free of parasites, no masses noted, decreased skin turgor, multiple puncture wounds at dorsum, most cranial wound has approx 2 cm pocket with purulent discharge CNS: mentation appropriate – no signs of neurologic abnormalities Assessment wounds- R/O bite wounds vs other underweight dehydrated mild anemia- R/O blood loss vs chronic disease vs other neutrophilia, monocytosis- R/O infection vs inflammation vs other Plan clipped and cleaned wounds, explored with hemostat and flushed with sterile saline already given convenia and SQ fluids at LVT intake Prognosis: fair SURGERY: Temporary waiver due to wounds, underweight, and anemia

S: patient is BAR, all wonds are dorsum are about 90% healed. Skin appears to be healing well, small scabs, one puncture is still open, however healing well and much smaller and very superficial. No discharge from open wound. Patient mm does seem a little palor O: T-101.2 P-188 R-28 A:Responsding Puncture wounds. P: Repeat PCV (28%) PROGNOSIS: GOOD
