Medical Summary 05/30/17 DVM Intake Exam Estimated age: 6 weeks Microchip noted on Intake? No. History : Subjective: Stray kitten, noted to have eye discharge. Observed Behavior – Very social, meowing, friendly, allowed all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR>240, RR=30, BCS=5/9. ORAL: WNL, all deciduous teeth erupted. EENT: Mild crusting and mucopurulent discharge, chemosis OU, worse OD. No nasal discharge. PLN: No enlargements noted H/L: Lungs clear, no murmurs/arrhythmias. No dyspnea. ABD: Mod. pot bellied appearance. No masses/organomegaly. U/G: Male, intact. MS: Ambulatory x 4, no lameness. INTEG: Full, clean haircoat. No skin lesions or fleas. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: 6 week old kitten with conjunctivitis, otherwise apparently healthy.. P: Applied Erythromycin ointment OU. Continue BID x 10 days. Temporary waiver from neuter surgery due to being underweight. Excellent prognosis.
