DVM Intake Exam

History : stray
Subjective: sever conjuctivitis OS, nasal discharge, sneezing.
Observed Behavior – allowed all handling
Evidence of Cruelty seen – n
Evidence of Trauma seen – n
P = 188 R = 28 BCS 4/9
EENT: OS – sever conjunctivitis, questible ruptured globe, no proptosis.
OD – mild conjuctivitis
Nasal – discharge, muco puralnet discharge
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: male, two testicles
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment – severe feline upper respiratory with questionable ruptured globe
Plan – take patient to isolation.
SURGERY: Okay for surgery
