Medical Behavior Evaluation BLUE
Medical Summary 05/25/17 DVM Intake Exam Estimated age: 4 yrs Microchip noted on Intake? No. History : Stray cat, suspected to be hit by car. Subjective: Observed Behavior – Lethargic, shivering, unable to fully gauge behavior due to injuries. Allowed all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – Yes. Objective: Lethargic, responsive when handled. Wrapped in warm towel to help dry him off and warm him up while being examined. mm=lt pink, moist, CRT<2 s. HR= 200, RR=28. BCS=4/9. ORAL: All adult teeth present, no obvious oral lesions or bleeding. EENT: No ocular discharge or chemosis. Moderate periocular swelling bilaterally. No nasal discharge but mild epistaxis and upper airway stertor. PLN: No enlargements noted. H/L: Lungs clear, purring, no murmurs/arrhythmias, no dyspnea. ABD: Non painful, no masses/organomegaly palpated. U/G: Male, intact, both testicles palpable. MS/INTEG: Slowly ambulatory x 4, no lameness. Prefers to remain in sternal recumbency in towel. No palpable limb fractures. Moderate, diffuse swelling over dorsum of head, maxilla and nasal region. No palpable crepitus or fractures of skull or mandible on gentle palpation. Mod. erythema, multiple abrasions over head, maxilla, upper lips, and nose. Several superficial abrasions over dorum of left hind paw, lateral thighs. No wounds appear deep or bleeding. Entire haircoat is soaking wet. CNS: Mentation quiet but appropriate, responsiveness improved as cat warmed up. A: Approx. 4 year old cat with head trauma, hypothermia. Clincally stable. P: Simbadol 0.45 ml SQ, Onsior 0.32 ml SQ, Convenia 0.34 ml SQ all adminsitered. 100 ml LRS SQ administered. Unable to obtain sufficient amount of blood for panel – reattempt when cat has been rewarmed fully and hydration improved. House in medical. Continue Simbadol 0.45 ml SQ q 24 hours x 3 more days. Continue Onsior pending Chem panel to assess kidney values. Continue 100 ml LRS SQ q 24 hours x 3 days unless eating/drinking well. Scheduled for sedation and skull radiographs tomorrow. Fair prognosis.
