Our vets have placed a catheter and started him on fluids but he would really benefit from a hospital setting

Progress exam
History: stray intake 1/21. Started on SQ LRS, cerenia, nebulization, doxycycline, terramycin, and buprenorphine for URI/dehydration.
Parvocite negative
CBC – dehydration, elevated lymphocytes, decreased neutrophils w/ bands noted
Chem/lytes – increased globulins, high normal tbili, sl elevated GGT, BUN 65
Subjective: QAR, ~8-9% dehydrated. Not interested in food. Very congested and resents handling, hisses and tried to flee. Diarrhea in cage and has urine scalding. Overall is declining. Continues to sit in urine and diarrhea reluctant to move.
Objective
EENT: mucoid d/c from eyes ou and purulent nasal d/c bilaterally, conjunctivitis, clean ears
Oral Exam: limited oral exam due to behavior
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic but audible upper resp congestion
ABD: Non painful, no masses palpated
U/G: MI, urine scalding on scrotum
MSI: ambulatory x4 but very weak, cachexia MCS 1/3, underweight, coat covered in diarrhea and has urine scalding on scrotum, megapaw RFL where IVC is placed
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment:
URI
Lingual ulcers
Severe dehydration
Underweight
Conjunctivitis
Dental disease
Cachexia
Hyperglobulinemia
Elevated GGT
Elevated BUN with normal creatinine r/o dehydration
Lymphocytosis, neutropenia
Prognosis: Poor
Plan:
re-wrap IVC and make sure still patent
Start IVF @ 150ml/kg/day
Continue doxycycline until 1/31
Continue buprenorphine until 1/24
Continue terramycin until 1/31
Continue nebulization until 1/26
Continue cerenia until 1/24
Recheck daily
If continues to decline, rec EHR
If diarrhea persists rec repeat parvo snap and metronidazole/fecal
