Medical Summary DVM Intake Exam Intact female Estimated age: 2 years Microchip noted on Intake? negative History : Stray cat. Coming from possible rabies positive comunity. Subjective: Aparently healthy pregnante intact female Observed Behavior – Patient was sedated. Before sedation the patient was stable. Evidence of Cruelty seen – NAF Evidence of Trauma seen – No trauma or wounds. Objective T = Not taken P = 160 bpm R = 30 bpm BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: NAF PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, fetus palpated, enlarged abdomen. U/G: Intact female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NAF Assessment pregnant Plan 0.1 5 ml telazol IM Prognosis: good. Observation for 10 days for signs of rabies Temporary waived for surgery due to rabies exposure.
