SAFE 4/6/18 BOWIE is a 4 yr old boy who is congested and needs further medical eval to see why. Please help this kitty get the care he needs.

History : stray
Subjective: BAR. Friendly and well behaved during PE, no signs of aggression. P struggled during med handling and a lot of the tech treatments had to be aborted. Audible upper airway congestion. p seems to breathe through open mouth. Tried to test nasal airflow, which seems to be negative bilaterally, but p did move around a lot. When I closed p’s mouth to test nasal airflow, p would begin to struggle so that he could breathe through his mouth. No ocular or nasal discharge.
Observed Behavior –
Evidence of Cruelty seen – n
Evidence of Trauma seen – n
P = wnl
R = wnl
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted. No nasal swelling noted, no pain on palpation of nose. Normal OU retropulsion. Normal tracheal palpation.
Oral Exam: nsf, no oral lesions or masses noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, referred upper airway noise
ABD: Non painful, no masses palpated
U/G: MI, 2 testicles
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: normal externally
absent nasal airflow bilaterally — r/o infection vs foreign body vs neoplasia vs granuloma vs anatomic abnormality
monitor and move to med ISO for now, presumptively treat for URI, until further work-up can be done
start doxycycline 1 mL PO SID x 10 days
nebulization 15 minutes BID x 7 days
tomorrow retest nasal airflow, CXR, skull, and neck rads. Concerned about sedating p given absent nasal airflow. P may need to be intubated.
