Medical Summary DVM Intake Exam Estimated age: 7-9 years Microchip noted on Intake? neg, placed History : brought in as a stray; sedated with hydro/dexdomitor IM for exam/wound debridement Subjective: QAR Observed Behavior – hisses in cage but allows handling Evidence of Cruelty seen – Evidence of Trauma seen – large wound on neck/face Objective BCS 3.5/9 EENT: OU-mild blepharospasm, mild mucoid discharge; corneas appear sl dull with mild opacity/scarring mid field stain neg OS, OD-mild superficial stain uptake in dorsomedial field ears clean no nasal discharge or crusting noted Oral Exam: mm lgt pk, tacky; CRT 2 sec (may be secondary to sedation); tip fractures of both upper canines with root exposure; missing most incisors; severe tartar build up on premolars PLN: No enlargements noted H/L: bradycardic secondary to sedation; no murmurs or arrhythmias appreciated ABD: non painful, doughy; no palpable abnormalities U/G: male intact MSI: ambulatory x 4; diffuse muscle loss large open wound under R side of chin and along neck; granulation tissue present over entire wound with thickened edges and some fibrin present CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Open wound Felv Anemia emaciated/dehydrated periodontal dz with fractured canines Plan PCV/TP-20/8.2; rec’d CBC/chem but were unable to get enough blood d/t dexdomitor and dehydration Cleaned wound, debrided edges while sedated convenia 0.4 ml SID gave cerenia 0.4 ml SID LRS 150 ml SID x 3 days buprenex 0.4 ml SQ or OTM BID x 4 days Prognosis: fair SURGERY: Okay for surgery
