I am a neutered male brn tabby domestic sh mix. The shelter thinks I am about 2 YEARS old.
I came in the shelter as a OWNER SUR on 07/01/2017 from NY 10032, owner surrender reason stated was PERS PROB. I came in with Group/Litter #K17-102810.

07/07/2017 Exam Type MS NEW URI – Medical Rating is 4 C – SEVERE CONDITIONS , Behavior Rating is NONE, Weight 12.8 LBS.
Hx: Urethral obstruction suspected on 7/5, pt sedated and bladder easily expressed with excellent stream and grossly visible hematuria. Started on 11ml/hr IVF, amoxiclav and prazosin. Pt was stressed and hunched in the back of his kennel 7/6/17; kennel paper was wet with either urine or water but pt didn’t urinate in litterbox. Today – litterbox is dry. S: Hiding in back of kennel under bogus paper. Flinches away from my touch but allows all handling and seems to appreciate nose and head rubs. O: BAR-H, BCS 7/9, MMs pink EENT: Mild serous nasal discharge, large superficial nasal planum ulceration. Clean ears and eyes. PLNs: Not enlarged. H/L: Sneezing. NSR, NMA. Eupnic, quiet lung sounds. Abd: Moderately tense. Difficult to palpate abdomen due to abdominal fat pad, but there appears to be a large soft bladder. M/S/I: Amb x4. No skin lesions noted. UG: Male neutered. No sign of pt licking penis, no penile spasms noted. Neuro: Alert and appropriate, no sign neurological deficiencies Pt chewed out IVC just after exam. Okay to leave it out. A: 1. FLUTD – unclear if pt is blocked or not. Definitely hasn’t urinated recently. UTI highly unlikely given age and sex. 2. Obese Short-term prognosis in the shelter: Poor. Long-term prognosis with treatment in a low-stress home: Good P: 1. Discontinue amoxiclav 2. Recommend drastically decreasing stress and increasing water consumption. Pt needs to be in a home environment more similar to his previous home. If he stays in the shelter, he will likely block in the near future. 3. LRS 300 ml SQ SID x5 days 1088

07/01/17 15:17 Basic: Blue is a 4 year old altered male domestic shorthair who was gifted to the owner from a friend. The owner has had Blue since Blue was a kitten. Blue has no known medical illnesses and his last visit to the vet was last summer at a mobile vet clinic. Socialization: Blue is shy and does not like to come around strangers. Blue has lived with children but he does not enjoy their company, if they come to close he attempts to swat them. Blue does not play with children or adults. Blue has lived with one other cat and the cat would try and play with Blue but he would swat at him. Blue is respectful of space around dogs and stays to his own side of the room when he is around a dog. Blue has never bitten or scratched another animal or person. Behavior: Blue can be left alone and everything will be exactly how it was left. When riding in a car, Blue likes the carrier to be on a lap and not a floor otherwise he struggles in the carrier. Blue does not like being bathed, having his nails trimmed, being picked up, he especially does not like being put in a carrier. For a New Family to Know: Blue is described as fearful and shy but affectionate when he warms up to someone. The owner’s favorite thing about Blue is that he is very reserved and calm. Blue loves playing with string or anything that dangles when he is in the mood to play. Blue is not a lap cat but he does like to be in the same room as the owner. Blue likes to sleep under the bed. Blue is fed Purina wet and dry, three times a day. Blue uses a hooded litterbox with clumping litter. Blue uses the scratching post on his cat condo to scratch. Upon intake, Blue was collared by owner and the picture was taken through the carrier. Blue’s pupils were dilated and his ears were back.

MEDICAL BEHAVIOR: 7/1/17- Sweet EVALUATION: Cage Condition: No change Reaction to assessor: Blue remains neutral, lying down on his cage bedding. Reaction to cage door opening: Blue becomes alert with eyes wide open, lip licking in place, ears erect and forward. Reaction to touch: Blue accepts the touch and head-butts the assessor’s hand and joyfully rolls over while appreciating the pets. Reaction to being picked up: He is calm when held, but then jumps back into the cage, while seeking attention again. BEHAVIOR SUMMARY: Average Blue interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.
