Medical Summary DVM Intake Exam Estimated age: yound adult, ~ 1-2 yr Microchip noted on Intake? negative History : owner surrender Subjective: BARH Observed Behavior – social, allowed most handling but hisses during PE when palp. tail/testicles and also very reactive to other animals or movement outside room door – hisses aggressively Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: nsf PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: apparently healthy Plan: no treatmenat at this time Prognosis: good SURGERY: Okay for surgery
