History: came in yesterday evening very cold and comatose. Provided warming and supportive care with enough good response to give him a chance overnight.
Subjective: BAR — active on exam and very friendly. Making biscuits repeatedly in cage. Not eating on own, but will eat when hand fed.
Objective:
T = 100.5
P = 140
R = 24
EENT: mild green ocular discharge OU, ears mild brown waxy debris, mucopurulent nasal discharge observed in both nostrils
Oral Exam: mild dental tartar/staining
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: tender, no masses palpated, tense abdomen
U/G: Intact male
MSI: Ambulatory x 4, no signs of trauma, unkempt haircoat that is dry/dull.
CNS: mentation appropriate – no signs of neurologic abnormalities
Prognosis: good
Plan: continuing with supportive care while at BACC. Recommend New Hope placement and further nursing care to ensure full recovery.
Provided 0.05 ml convenia SQ at time of exam and 0.2 ml doxycycline (50mg/ml) po and nutrical po. Adding 0.2 ml doxcycline (50mg/ml) po q24 for 10 days to treatment as well as as 30 ml sq LRS q24 for 3 days and nutrical po bid for 5 days.

U/G: Intact male
MSI: matted haircoat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment: Hypothermic and failing to thrive
Prognosis: poor to fair (depending on response to therapy)
Plan: Warming up and providing supportive care. Responding well as of 7:00pm. Set up in cage with warm support. Provided 25ml LRS SQ and 3 ml 50% dextrose SQ. Nutrical and Royal Canin kitten food provided. Reassess in the morning.
