Medical Summary DVM Intake Exam Estimated age: 5-6 weeks Microchip noted on Intake? no History : not known Subjective: QAR, eating on own Observed Behavior – sweet, allowed all handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 4/9 EENT: Moderate mucopurulent discharge and conjunctivitis OU, marked synblepharon OS, ears clean, no nasal discharge noted Oral Exam: all deciduous teeth present, no ulcers PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male, two testicles palpable MSI: Ambulatory x 4, marked flea infestation, no masses noted, diffusely unkempt haircoat CNS: mentation appropriate – no signs of neurologic abnormalities Very faint FeLV positive on Combo test Assessment: underage kitten with conjunctivitis/URI, flea infestation, eating on own, faint Combo FeLV positive – true vs. transient infection. Plan: Start erythromycin ointment OU BID x 10 days and doxycycline 0.1 mL PO SID x 10 days. Gave Capstar and Frontline at intake. Seek New Hope/foster placement. Retest FeLV at 6-7 months. Prognosis: guarded if true FeLV positive SURGERY: Temporary waiver due to URI/underage
