History : Subjective: Observed Behavior – very vocal, meowing constantly, purring and sweet on exam Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = — P = — R = — BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: deciduous dentition present, oral cavity WNL PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty haircoat, no alopecia CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment ~4 week old female intact kitten dirty haircoat underage/underweight eating well on her own Plan Recommend New Hope placement Prognosis: Good SURGERY: Temporary waiver due to underage/underweight
