Medical Summary DVM Intake Exam Estimated age: 5 weeks Microchip noted on Intake? neg History : suspected high rise kitten Subjective: Observed Behavior – allows all handling Evidence of Cruelty seen -no Evidence of Trauma seen – yes Objective P = 240 R = eupneic, meowing BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted. Small amount dried blood by nares but no occlusion to breathing Oral Exam: no oral lesions, pink gums PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, grossly normal MSI: Ambulatory x 4, with possible limp on R FL, no palpable fx’s or instabilities on any limbs, no spinal pain detected, no pain detected skin free of parasites, no masses noted, healthy hair coat. No palpable fx’s or swelling on face CNS: mentation appropriate – no signs of neurologic abnormalities Assessment r/o hise rise trauma, with possible trauma to R FL: r/o nerve trauma, musculoskeletal, other Plan buprenophine 0.3mg/ml: 0.02ml at side of cheek BID x3 full body radiographs- open growth plates, otherwise no obvious fx’s or dislocations assist feeding monitor app Prognosis: good SURGERY:n/a- kitten too young, temporary waiver
