Hello, my name is Beluga. My animal id is #10982. I am a desexed male gray cat at the Manhattan Animal Care Center. The shelter thinks I am about 5 years 2 weeks old. – P
I came into the shelter as a stray on 27-Oct-2017, with the surrender reason stated as person circumstance- death of owner.
Beluga is at risk for medical reasons, having contracted URI, which is contagious to cats. Beluga is not recovering well in the shelter and her course of medication has just had to be extended beyond the usual 10 days for this conditions.

No History : Owner passed away a few days ago, sibling would like to take cat home Subjective: QARH Observed Behavior – slightly tense body posture, allowed all handing though started to vocalize at end. Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective T = P = 200 R = 28 BCS = 5/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: mild tartar PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Neutered male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: not done Assessment healthy Adult Prognosis: Excellent Plan: No medical plan needed

Recheck of URI day 7: S/O: QAR mild dehydrated – decreased appetite – mild nasal discharge/sneeze – congested – no oral ulcers observed A: URI persists, dehydrated, decr. appetite P: cont. doxy + give Cerenia 0.45 ml SQ SID x 3 days + 150 ml LRS SQ x 3 days + 1/4 tab Mirtazipine 15 mg PO + recheck
Recheck URI, day 8 doxy; started on cerenia, LRS and mirtazapine yesterday S/O -QAR -severe nasal congestion -no significant appetite -appears eupnic A URI with severe congestion P dexamethasone SP 4 mg/ml: give 0.3 ml SQ once Continue other treatments Recheck in 2 days
Hx: Today is day 10 of doxycycline for URI. Mirtazapine given 11/11/17. Pt was heavily congested on 11/12/17 so pt received dexamethasone. Today, pt is still congested, with mild serous ocular discharge. Extend doxycycline x5 more days. Add nebulization x5 days. Add lysine x10 days. 1088

Basic Information:: Cat allowed handling Easy into kennel.
KNOWN HISTORY:: Beluga was brought in as a stray, so we cannot speak to his behavior in his previous home.
MEDICAL BEHAVIOR:: Observed Behavior – slightly tense body posture, allowed all handing though started to vocalize at end.
ENRICHMENT NOTES:: 11/1/17 Resting at front on approach, soft eyes and body. Started purring and rubbing against the bars when spoken to. Leaned and arched enthusiastically into pets, purred louder, age offered treats. 11/2/17 Sitting up at front, soft eyes and body, looking around. Slow blinked when spoken to. Stood when door opened, leaned and arched enthusiastically into pets, purred loudly. 11/5/17 Diagnosed with URI. Dozing on bedding towards front, woke on approach. Lay in place, soft eyes and body, started purring. Stood when door opened, head-butted, leaned and arched into pets, purred louder. Ate offered treats.
