History : Finder has been feeding this cat recently and suddenly noticed that the cat’s back left leg is swollen and pt cannot walk on it
Subjective: Alert, mews
Observed Behavior – Slightly tense. Allows some handling but yowls on palpation of LHL.
Evidence of Cruelty seen – None
Evidence of Trauma seen – Left hind leg is severely swollen
Objective
QAR, estimated 8% dehydrated, BCS 4/9
EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: Clean adult teeth
PLN: No enlargements noted
H/L: NSR, NMA Lungs clear, eupnic
ABD: Soft, non painful, not distended, no masses palpated
U/G: Male intact, testicles S/S
MSI: Not weight-bearing LHL. Left hind leg distal to the hock is severely swollen, with small vesicles visible between the toes, and a small abrasion on the metatarsal paw pad. No puncture wounds are noted. No laxity or crepitus noted. Skin free of parasites, no masses noted, healthy hair coat.
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
Assessment: LHL severely swollen – R/O abscess/cellulitis from bite wound or other cause, vs. fracture vs. other
Prognosis: Good
Plan:
1. IVC, run IV LRS at 18 ml/hr
2. Clavamox 0.7 ml PO BID x10 days
3. Simbadol 0.4 ml SQ SID x7 days
4. Onsior 0.2 ml SQ (1.3 mg/kg) today and tomorrow; consider increasing dose to 0.3 ml (2 mg/kg) after pt is rehydrated)
5. LHL rads tomorrow – sedate with 0.15 ml Dexdomitor and 0.05 ml butorphanol IM
