SAFE 7/2/18 BARBOSA is a 10 year old neutered boy who was brought in to the shelter as a stray. He needs further medical eval to determine if he has a UTI or some renal issues. He also needs follow up dental care. Please give this handsome ginger boy a new home where he can get all the care he needs.

Objective
BAR, MMs pink, BCS 3/9 with moderate muscle wasting
EENT: Eyes clear, mild discharge AU, no nasal or ocular discharge noted
Oral Exam: Severe tartar and gingivitis, especially on PMs
PLN: No significant enlargements noted
H/L: NSR, NMA, Lungs clear, eupnic
ABD: Soft, non painful, not distended, no masses palpated
U/G: Male neutered
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Normal externally
CBC/chemistry:
Hct 34%, WBF 12.57 k/ul, plt 25 k/ul – plt clumps detected
Elevated globulins (5.5 g/dl). Glucose 88, BUN/creat 32/1.6, normal electrolytes.
T4 1.6
Assessment:
1. Underweight and muscle wasted
2. Dental disease
3. Azotemia – R/O pre-renal vs. renal
4. Elevated globulins – R/O dental disease vs. UTI vs. other
Prognosis: Fair-good
Plan:
1. Recommend urinalysis, ideally by cysto
2. Recommend dental cleaning
