Medical Summary DVM Intake Exam Estimated age: 1yr Microchip noted on Intake? negative History : o surrender because o is travelling Subjective: sweet temperment Observed Behavior – docile, allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 150 R = 24 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild gingivitis, chipped R maxillary canine-non painful PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: MI, both testicles descended MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment MI mild periodontal dz Plan ok for adoptions and neuter long term rec brush teeth with cat toothpaste and brush if p tolerates if ever undergoes a dental px, rec dental rads to determine vitality of chipped tooth, was non painful when touched on exam, likely doesn’t bother p, can be monitored Prognosis: good SURGERY: Okay for surgery
