Hx: Suspect HBC, was treated with Onsior and Simbadol
S: Alert, looks around, allows handling
O: QAR, BCS 5/9, MMs did not assess due to blood in mouth
EENT: OS proptosed globe, buphthalmia, hyphema. OD scleral hemorrhage, suture lines visible, normal fundic exam. Blood in mouth and on nasal planum, pt avoids oral exam and touch to nasal planum.
H/L: HR 20/min, increased inspiratory effort. NSR, NMA.
Abd: Soft, nonpainful, no masses
M/S/I: Blood on front paws from face, no other injuries noted
Left lateral thoracic rad – no sign of pneumothorax or lung consolidation
A: Trauma to face from collision with car
Prognosis: Fair. OS appears proptosed and will need to be enucleated. Pt may need the jaw wired shut to manage suspected mandibular fracture. Pt is in a large amount of pain but pt appears to be alert and may be able to survive these injuries.
P:
1. Placed IVC, started LRS at 15 ml/hr
2. Continue Simbadol and Onsior
3. When pt is more stable, rec sedated skull rads to evaluate for fractures

OS partially proptosed
Observed Behavior -allows exam, purring despite injuries
Evidence of Cruelty seen -n

R =stertor, referred upper airway noise
BCS 5/9
EENT: OS bupthalmic, hyphema, ears clean, nose caked with blood, interfering with respiration.
Oral Exam: jaw stuck open skewed to patient’s right–assumed rani fx r side
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs referred upper airway noise
ABD: Non painful, no masses palpated
U/G: intact male
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment: hit bay car, jaw fracture, OS severe injury
Prognosis:fair–if no other injuries discovered prognosis is good
Plan:radiographs to assess to further fractures and assess jaw.
will need NH placement and jaw repair. probably enuclation os
gave 0.5ml simbadol and 0.35ml onsior (note these doses are high–based on 8 #)
