Medical Summary DVM Intake Exam Estimated age: 5 weeks Microchip noted on Intake? scan negative History : stray Subjective: Observed Behavior – friendly, eating canned food well after exam Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective BCS 3/9 EENT: OU- elevated third eyelids, conjunctival hyperemia and discharge, ears clean, nasal congestion and mild discharge Oral Exam: deciduous teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, small patches of slopecia at distal forelimbs with crusted material CNS: mentation appropriate – no signs of neurologic abnormalities Assessment conjunctivitis nasal congestion- R/O URI vs other underweight patches of alopecia and crusted material- R/O debris stuck to fur vs dermatophytosis vs other Plan wood’s lamp: negative monitor skin and recommend fungal cutlure if nto improving/getting worse start erythromycin OU BID x 10 days start doxycycline 4 mg PO SID x 10 days Prognosis: fair SURGERY: Temporary waiver due to URI and underage
