Medical Summary DVM Intake Exam Estimated age: 10 mo Microchip noted on Intake: Positive:981020023063845 History: Stray Subjective: Observed Behavior – BAR Evidence of Cruelty seen – None Evidence of Trauma seen – None (small laceration at the surgery site) Objective: Hydrated; T =NT P = 180 R = 34 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: No dental tartar, all permanent teeth are present, MM Pink and moist, PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Spayed female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, small wound/laceration at the surgery site CNS: Mentation appropriate – no signs of neurologic abnormalities (full neurologic exam not performed) Rectal: Not performed Assessment 1. Small wound/laceration at the spay area 2. Apparently Healthy Plan/Treatment/Recommendations 1. Clean with Chlorexidine and Apply TAB topical BID 5 days Prognosis: Good Monior while in BACC
