Medical Summary DVM Intake Exam Estimated age: 2 yrs Microchip noted on Intake? no History : O/S due to moving to an apt that does not allow pets. Subjective: Cat is Q/BARH. Observed Behavior – At back of cage behind litter box, will peek out and quickly looks back. A little shy/timid at first, but warms up quickly. Little restraint required for examination. Curious, willing to explore the environment, able to be focused back on people. Allows all body parts to be touched. Startled by quick movements or noises, but recovers quickly. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 200 R = sniff BCS 3-4/9 EENT: Eyes – mild epiphora (wnl for breed), ears – mild amount brown waxy debris, no nasal discharge noted Oral Exam: Grade 1/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: wnl MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat – freshly groomed CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: On the thin side, otherwise Apparently Healthy Plan: Acceptable candidate for adoption or rescue. Cleaned AU – monitor for further debris. Monitor appetite. Prognosis: Good-Excellent SURGERY: Already neutered
