Hx: developed a URI post adoption and was taken to outside veterinarian; blood work there showed elevated amylase and elevated t. bili (0.6), owner surrendered back to MACC for further treatment; was started on doxycycline, nebulization but has been lethargic, inappetanct in iso for 48 hours S/O BAR, appears much more interactive and alert outside of cage mm pk, tacky; CRT <2 sec hypersalivation mucoid nasal discharge, sneezing, nasal congestion OU-mild blepharospasms eupnic, heart/lungs WNL soft, nonpainful, doughy abdomen with no palpable abnormalities mild diffuse muscle wasting repeat bloodwork available for review CBC-mild non regenerative anemia (27%), high normal neutrophils, thrombocytopenia (suspect lab error) Chemistry-low BUN (secondary to inappetance), mildly elevated globulins A URI Inappetance-r/o secondary to URI vs other systemic illness Mild anemia-r/o secondary to URI vs other Mild muscle loss P mirtazapine 15 mg tab: 1/4 tab PO q3d dexamethasone 4 mg/ml: 0.2 ml SQ continue cerenia, LRS, doxycycline and nebulization therapy if no improvement in appetite, rec’d IVF rec’d rechecking CBC in 7-10 days prognosis: fair to good; suspect inappetance and anemia may be associated with URI but she does have some mild muscle wasting that may indicate another underlying systemic issue

09/14/17 16:27 Basic information: Arabella is a 3 yr old cat who was gotten from ACC and was kept in the same home for about a week. Unfortunately due to medical problems she could not be kept. No records of her ever going to the evt. Socialization: Around strangers Arabella is described friendly/outgoing, relaxed and respectful and plays gentle. She was not socialized with dogs, cats or children and there’s no records of her ever biting another animal or person. Behavior: Arabella will hide in fear and will sleep during car rides. You will struggle with Arabella when attempting to pick her up, and isn’t bothered with being placed in a carrier and being disturbed while sleeping. For a New Family to Know: Arabella is described as friendly, affectionate, mellow and quiet. She has a low activity level, enjoys cuddling and will follow you around when home with you. She has been kept mostly indoors, prefers to sleep on her owners bed, ate friskies wet food 3x a day and loved the greenies dental treats. She is considered to be litter box trained, prefers an uncovered litter box with clumping litter and didn’t have an interest in scratching posts.

ACTIVITY LEVEL: Laid back VOCAL: Quiet CHARACTER TYPE: Calm, Sweet KNOWN HISTORY: Lived Indoors Previously lived with: adults Behavior toward strangers: sweet and affectionate MEDICAL BEHAVIOR: 9/12/17- Subjective: Alert, allows all handling ENRICHMENT NOTES: 8/31/17 Resting in top cubby, looked up and became alert on approach, eyes dilated. Seems conflicted. Head-butted, leaned into pets, and ate offered treats, but tense posture and loud grumbling throughout interaction. Came out to main compartment for more treats after door closed. EVALUATION: Cage Condition: No change Reaction to assessor: Arabella remains neutral, lying down with head up on her cage bedding during the approach. Reaction when softly spoken to: Arabella remains immobile in place. Reaction to cage door opening: Arabella becomes alert with soft eyes, ears erect and forward. Reaction to touch: Arabella slowly leans into the assessor’s hand while she remains resting, but seems to be conflicted and low grumbles after a few strokes. Reaction to being picked up: She becomes antsy then jumps back in to the cage. BEHAVIOR SUMMARY: Average Arabella interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

Scanned positive History : Adopted, was seen at private vet clinic for not doing well and the private vet suspected pancreatitis due to elevated amylase and PrecisionPSL. Bloodwork also showed mildly elevated WBC (18.8), neutrophils (15.6K/ul), mildly elevated T. bili (0.6), glucose (211). Subjective: Alert, allows all handling Objective BAR, estimated 5-8% dehydrated based on decreased skin turgor, MMs pink, BCS 5/9 EENT: Mild serous ocular discharge. Mild hemorrhagic to mucoid nasal discharge. Oral Exam: Clean teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Referred upper resp sounds but quiet lungs. ABD: Non painful, no masses palpated U/G: Female, spayed recently, spay incision is healing well with no swelling, discharge or erythema MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment: URI Plan: 1.
