Hx: Hit by car, traumatic spinal injury with hindlimb paraparesis; mild degloving injuries over hindfeet
PE
EENT: Eyes clear, ears clean, no nasal discharge noted
Oral Exam: wnl
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, large urinary bladder
U/G: intact male
MSI: Ambulatory x 3 paraparetic in hindlimbs, worse in RHL, muscle wasting in hind, skin free of parasites, road rash with mild degloving injuries over hindpaws
CNS: mentation appropriate, hindlimb paraperesis – withdrawals intact, proprioception absent RHL, large urinary bladder- r/o UMN bladder
A: traumatic disc injury with hindlimb paraperesis, r/o UMN bladder- pt urinated in cage but did not empty bladder, easily expressed manually. Road rash healing appropriately

continue simbadol 0.35 ml SQ SID
dexemethasone injection IM SID x 3d
famotidine injection SQ SID x 3d
rec’d MRI with possible hemilaminectomy after placement
prognosis: guarded esp if unable to urinate; fair prognosis with surgery if neuro status does not decline
Re-exam 9/21/17:
Sedated with 0.1 ml dexdomitor/0.1 ml ketamine for radiographs
Full body radiographs: traumatic, severe collapsed disk space at L3/L4; copious amount of fecal material in colon; mild diffuse bronchiole pulmonary pattern; NOSF

had one dose of onsior today, consider giving steroid injection tomorrow vs continuing onsior
