Medical Summary UPDATE – 1/5/16 1:30pm: CBC shows anemia, mild leukocytosis with likely stress leukogram and “bands suspected” which could be bands or mis-read NRBCs (retic count is 79.6 K/ul or 1.5%), and thrombocytopenia (possibly from clotting). Chem is all WNL including total protein (6.3) and calcium (9.7). Calcium supplementation not indicated at this time, and no hematological signs of retained fetus. Blood loss at parturition may be responsible for current illness. Continue supportive care (SQF, food) and seek placement asap. 1088 ——————– 1/5/16 10:18am: Sex: Female, gave birth to four live kittens yesterday Scan: Negative Estimated age: 1.5 years old based on teeth and size Flea dirt: moderate flea dirt Hx: Gave birth to four apparently healthy live kittens (two M and two F) evening. Today pt has diarrhea, and vomited during admission. S: Lies down or sits in cage, looks around during exam while nursing, tolerates all handling, minimal restraint needed, pt not currently trying to flee O: BAR, BCS 4/9, MMs pale pink and moist, mildly delayed skin turgor (5-8% dehydration) EENT: No discharge OU, AU, nose. Clean teeth. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. Diarrhea staining on hind legs and both flanks UG: Female. Slightly firm thick uterus in caudal 1/3-1/2 of abdomen, no pain on palpation. Mammary glands enlarged (currently nursing). Vulva normal in size. A: 1. Recent parturition (1/4/16 in evening) – four live kittens – very late in the year to be queening 2. Diarrhea – R/O metabolic disturbance secondary to parturition (ex: hypocalcemia) vs. post-obstruction diarrhea after parturition vs. infectious disease (ex: parvo) 3. Vomiting – R/O metabolic disturbance secondary to parturition (ex: hypocalcemia) vs. post-anorexic vomiting after brief starvation vs. infectious disease (ex: parvo) 4. Mild dehydration 5. Slightly underweight Short-term prognosis: Fair P: 1. Parvo test – NEGATIVE 2. LRS 150 ml SQ 3. CBC and chem, calcium supplementation if necessary (calcium gluconate 100 mg/kg IV SLOWLY, monitor resp/heart rate during administration) 4. Gave pyrantel 0.9 ml PO during exam 5. Wait at least 3 days for vaccines due to unstable condition 6. Free access to high-calorie food and water 7. Consider rads to confirm all kittens born if signs not resolving 1088
